Sixth Form Intake Information 2026

The information requested in this form is used to support the Sixth Form Coordinator and school staff in working with the student and communicating with parents. All details will be stored electronically and handled with strict confidentiality.

  • Parent/Carer details - Main Address

    1. Parents living abroad must nominate a guardian living in the UK with whom the pupil would normally spend exeat weekends. 2. It is helpful to know the rank of members of HM forces, although this will not be used in correspondence to a private address for security reasons.
  • Name & address of parents with whom the pupil normally resides. Please note that the parents will receive all reports, accounts and notices (unless ticked in the Second Address box).
